Just happened to notice when I was a little bit sloppy removing an item...I unequipped an item and dropped it in the little area of the inventory panel between the main pack items and the backpacks. Even though that area is within the boundaries of the inventory UI, it dropped the item on the ground! Further experimentation showed that it happens for some areas of the UI, but not others. Just wanted to give an FYI to be extra careful until that's fixed.
